UPDATE YOUR SIGNATURE

When you vote by mail, your signature is used to verify your identity.

It's a good idea to send a signature update to the Supervisor of Elections. 

  1. Print this form (click here) 
  2. Check the "Record Update" box 
  3. Complete lines 4, 5, & 6 and sign in the signature box at the bottom 
  4. Mail the form to Supervisor of Elections, P.O. Box 1460, Bartow, FL 33831
